What Is Hidradenitis Suppurativa? Don't Ignore Those Lumps

It’s definitely not an easy term to pronounce hidradenitis suppurativa (HS) nor is it an easy condition to live with. Up to four per cent of Canadians are affected by this painful skin disease. However, dermatologist Dr. Afsaneh Alavi, a director of the Wound Healing Fellowship at Women’s College Hospital in Toronto, says diagnosing the condition can often be delayed or is misdiagnosed, making it difficult to adequately assess the true prevalence of the disease.

So, what is hidradenitis suppurativa?

Hidradenitis suppurativa is a long-term skin condition that features small, painful lumps under the skin. Despite also being known as acne inversa, this condition is not a form of acne. The lumps typically develop where the skin rubs together, such as the armpits, the groin, between the buttocks and under the breasts. The lumps may break open and smell or cause tunnels under the skin, according to Dr. Alavi. With time, the lesions can be clustered with residual scarring.

I want young people who have recurrent boils in areas with sweat glands (armpit, groin, buttocks, under breasts, etc), to speak with their doctor right away as there is treatment. I want them to know that it is not their fault, and they shouldn’t blame themselves. HS is not caused by poor hygiene, nor is it a sexually transmitted disease or contagious, it is genetic. Don’t be affected by the stigma. If you have HS, talk about it. There is a lot of support out there.

What can hidradenitis suppurativa look and feel like?

Hidradenitis suppurativa often starts with a single, painful bump that persists for weeks or months. For some people, the disease progressively worsens and affects multiple areas of their body. Hidradenitis suppurativa lumps may look like the following:

Blackheads: Small pitted areas of skin containing blackheads — often appearing in pairs.

Red, tender bumps: These bumps often enlarge, break open and drain pus, which can have an odour. The bumps may itch and burn where skin rubs against skin.

Painful, pea-sized lumps: These hard lumps, which develop under the skin, may persist for years, enlarge and become inflamed.

Tunnels: Over time, tracts connecting the lumps may form under the skin. These wounds heal very slowly, if at all, and can leak pus.

Who gets them?

Hidradenitis suppurativa most commonly occurs in women between the ages of 20 and 30. The ratio of female to male is three to one, says Alavi.

The condition can be associated with several other conditions, including arthritis, severe acne, obesity, inflammatory bowel disease, Crohn’s disease, metabolic syndrome and diabetes. And just another reason to ditch the cigarettes: smoking tobacco has been linked to hidradenitis suppurativa.

What causes it?

Dr. Alavi says that while the causes are mostly unknown, it is thought that HS is mainly genetic.

“There is some research that shows HS is a disease of hair follicles that has genetic and environmental causes,” she says. “With heat, the pore can get clogged and a cyst will form, holding a collection of fluid, within a closed area.”

The doctor adds that bacteria then accumulates in this environment and eventually the cyst ruptures and causes inflammation.

“Most of us will get ingrown hair, but people with HS are prone to chronic inflammation and recurring boils, which are painful and cause scarring,” Dr. Alavi told HuffPost Canada.

The doctor says that HS is not caused by poor hygiene, nor is it a sexually transmitted disease or contagious.

When should you go to the doctor?

Patients often suffer for years without effective treatment, leaving them frustrated in addition to helpless and ashamed, said the dermatologist. It is so important that people know about this disease, and to get early medical attention, she adds.

“The average Canadian (HS) patient will see five doctors with over 17 visits spanning eight years before being diagnosed,” says the Canadian Hidradenitis Suppurativa Foundation website.

Early detection is key to getting effective treatment and you should see your doctor if your condition is painful, doesn’t improve in a few weeks, returns within weeks of treatment, appears in several locations and recurs often, according to the Mayo Clinic.

“If diagnosed earlier, treatment can be started right away and potentially lessen the damage that people with this condition often experience,” says Alavi.

If young people notice that they are developing recurrent boils at least twice in a six month period they need to speak to their doctor. Dr. Alavi says that people with HS tend to suffer from the disease for seven to nine years before being diagnosed and “this is unfortunate because of the physical and psychological impact of the disease.”

And, pay attention to any new signs or symptoms as these may indicate either a recurrence or a complication of treatment.

What will a doctor do to treat it?

Depending on the severity of the disease, there are a few different treatment options. As the disease is inflammatory, in most cases patients will need both medical and surgical interventions. Medical interventions can include systemic antibiotics, topical treatments, anti-androgen (hormonal) treatment, and biologic treatment with Adalimumab the only Health Canada-approved medication for the skin disease, says Alavi.

How can you manage it?

Patients play an important role in the management of chronic disease, particularly HS, says Dr. Alavi. They can detect flare-ups and seek treatment right away.

“It is also important that patients recognize triggers that can cause flare-ups, such as wearing tight clothing, shaving, smoking and gaining weight,” she says.

It has been shown that smoking and obesity play a role in flare ups of the disease. Some patients also find that removing dairy from their diet helps, but there is no research to support this.

There’s no cure for HS, so you’ll have to manage the condition for life.

East
In four games for the Warriors, Yudin was nearly perfect at the dish going 13-for-14 (.929) and also went 2-0 with a 0.64 ERA in the circle. She recorded two doubles, two triples a 1.357 slugging percentage and .933 on base percentage. Yudin, a junior, registered four multiple-hit contests, including two 4-for-4 outings against Carlisle County (2B, RBI, R) and Ballard Memorial (2B, 3B, 3R). She also went 3-for-3 with a double and three RBI against North. In the circle, she earned a pair of complete-game victories, including a one-hitter (1 R, 0 ER) with seven strikeouts against Carlisle. Yudin also struck out six and allowed an earned run on four hits in the six-inning victory over Ballard.

North
Berghoff, a senior captain, also put in a near perfect performance last week for the Dolphins. She was 9-for-10, reached base safely in 13-of-14 plate appearances (.929 OBP) and slugged an astronomical 2.100 with two doubles, two triples and two long balls. She also scored 11 out of the 13 times she reached base and knocked in eight runs. In a doubleheader against Tri County Area, Berghoff was 6-for-6 with two doubles, a home run, six RBI and nine runs scored. She scored a five times and recorded four RBI in the opener. Against Andrew, she doubled twice, homered, scored twice and knocked in two runs.

South
Watkins was 5-0 with 33 strikeouts and did not allow a run in five appearances (17 IP) and also .600 (9-for-15) with four doubles, a homer and 14 RBI for the Golden Tigers. She opened the week with a two-hit shutout of Elmore County in which struck out 12 and did not issue a walk. Over the course of her other four appearances (11 IP), she allowed just two hits, struck out 21 and walked five. Watkins finished the week with her opponents batting just .073 against her. At the plate, she went 3-for-4 with a double, home runs, five RBI and two runs scored versus Elmore Country. Watkins also knocked in three runs in wins over Charles Henderson and Zion Chapel.

South Central
Boles, a senior right-hander, tossed a perfect game last week for the Bulldogs and finished the week 2-0 with a 0.00 ERA and 21 strikeouts in 13 innings of work. In the perfect game, she struck out eight Newton High School batters as Pollok went on to twin 14-0. Boles continued her dominance and was the hero on Friday against Garrison. She twirled a two-hit complete-game shutout and hit the walk-off two-run home run in a 2-0 eight-inning victory, Boles allowed just two singles, struck out 13 and did not walk or hit a batter to help the Bulldogs improve to 18-5.

West
Young batted .714 (10-for-14) and slugged a whopping 1.643 with 14 RBI, 10 runs scored and a .867 on base percentage to lead Paradise Honors to a 3-0 week. The sophomore captain recorded three doubles, two triples and two home runs. She knocked in a season-high seven runs by going 4-for-5 with a double and home run against Tempe Prep. Young homered, doubled, knocked in three and scored three times versus Chandler Prep. She capped off her dominant weekend by going 4-for-4 with a double, two triples and four RBI against Ajo.

Will the death toll rise? 

Almost certainly. Emergency responders are struggling to reach parts of the three countries after some roads, bridges and communications networks were washed away or destroyed.

The city of Beira with its some 500,000 residents is said to be 90 percent destroyed, while some 152 square miles have been flooded. 

Mozambique’s president, after flying over the region, has warned as many as 1,000 people could die. But further deaths could occur if people clinging to hope are swept away by the rising waters or are not reached in time with critical water, food and other aid.

And now authorities are warning of water-borne diseases after what infrastructure existed in the largely impoverished countries was stripped away.

What is being done to help?

Mozambique and Malawi are two of the poorest countries in Africa, and the economy of once-prosperous Zimbabwe has collapsed in recent years.

Aid appeals for millions of dollars have begun as the United Nations and other humanitarian organizations rush to deliver food, water, fuel and medicines to the vast affected area. 

The European Union and Britain have been among the first to pledge aid. The EU says its initial package to Mozambique, Zimbabwe and Malawi is worth about $3.9 million, while Britain says it will provide up to $7.9 million (£6m).

The US Embassy in Zimbabwe says it is "mobilizing support" for partners in the three countries, with no further details.

Tanzania’s military airlifted some 238 tons of emergency food and medicine to the three countries. Zimbabwe’s president has said a number of countries, including the United Arab Emirates, South Africa, Botswana, Namibia and Angola, also were offering aid.

Aid organizations and authorities are rushing humanitarian aid to Beira and other areas by air and sea, while rescue efforts push on.

The torrential rains are expected to continue in central Mozambique until Thursday. As the water begins to recede throughout the region, the extent of the devastation will become clearer.

And thousands, perhaps scores of thousands, of families could be on the move, seeking new refuge after their homes were washed away. "I salvaged nothing except this baby," Chipo Dhliwayo told AP in eastern Zimbabwe, referring to her 6-month-old son.

‘Egg Boy’ defends egging far-Right Australian senator in wake of Christchurch shooting

An Australian teen known around the world as "Egg Boy" conceded on Monday that egging a far-Right senator was not the right thing to do, but said the gesture united a world reeling from a white supremacist’s alleged massacre of 50 Muslims in New Zealand.

Will Connolly, 17, gave his first television interview since becoming an online hero among many for cracking an egg on Fraser Anning’s head as the controversial politician spoke at a news conference after a gunman killed or wounded 100 worshippers at two Christchurch mosques on March 15.

Mr Anning has been widely criticised for blaming Muslim immigration for the racist attacks.

Mr Connolly said he was embarrassed that the international attention he had attracted with the egging, which was caught on video, had distracted attention from the victims in Christchurch.

"I understand what I did was not the right thing to do, however this egg has united people and money had been raised – tens of thousands of dollars has been raised for those victims," Mr Connolly told Ten Network television’s The Project programme.

A GoFundMe page set up to raise A$2,000 (£1080) to pay for Mr Connolly’s "legal fees" and "more eggs" reached A$80,000 on Monday.

Mr Connolly said the money would go to Christchurch victims.

He said he hasn’t had time to think about how he will respond to offers from supporters such as a vacation in Turkey and a lifetime of free beer in Canada and Wales.

Australian Brenton Tarrant was arrested within an hour of the mosque shootings and has been charged with murder.

Mr Anning came under blistering criticism over tweets within hours of the massacre, including one that said, "Does anyone still dispute the link between Muslim immigration and violence?"

"The real cause of the bloodshed on New Zealand streets today is the immigration program which allowed Muslim fanatics to migrate to New Zealand in the first place," he said in a later statement.

Police say the egging constituted an assault. But police are also investigating Mr Anning, who retaliated by striking Mr Connolly twice, and Mr Anning’s supporters who roughly pinned the boy to the ground. No charges have yet been laid.

Mr Anning later defended his actions, telling reporters: "He got a slap across the face, which is what his mother should have given him long ago, because he’s been misbehaving badly."

Mr Connolly said his mother had not approved of the egging.

"There’s no reason to physically attack anyone. She’s glad I stood up for what I believe in; she definitely disagrees with the way I did it," Mr Connolly said.

Tim Allen has revealed that Keanu Reeves plays a role in Toy Story 4 – but we don’t know much about it besides the fact that he was originally too similar to Buzz Lightyear.

Speaking on The Tonight Show, the Buzz Lightyear actor said Pixar “will surprise you with the toys they came up with. Characters came back, new guys in it that are great. Keanu Reeves has got a great part.”

“Actually, a little inside story. Even [Reeves] said, gentle, wonderful guy that he is, ‘This sounds too much like Buzz Lightyear.’ And his character does have an edge to that. And the guy said, ‘So we calmed him down a little bit,’.”

It’s not clear exactly who the Matrix and John Wick actor will be playing, but Allen indicated that he’d be playing a particularly small toy – around the size of the toy soldiers already in the film.

Reeves isn’t the first surprise star addition to the cast. Tony Hale was revealed as Forky in the film’s first teaser, before a second trailer showed off comedians Key & Peele as Ducky & Bunny..

British-funded dengue forecasting system in Vietnam aims to curb mosquito-borne virus

A British consortium funded by the UK Space Agency on Tuesday launched a pioneering dengue forecasting system in Vietnam, in the hope of curbing the spread of the deadly mosquito-borne disease that has afflicted the south east Asian nation for generations.  

The “D-MOSS project” led by HR Wallingford, a British civil engineering and environmental hydraulics consultancy, uses a combination of satellite information, weather forecasting, and historical data on previous outbreaks to create an early warning system for potential dengue danger zones.

The combined information would allow communities and health authorities to prepare for and try to prevent a dengue outbreak several months in advance, Gina Tsarouchi, a senior engineer at HR Wallingford told The Telegraph. “It’s the first [of its kind] in the world,” she added.

Transmitted mainly by the aedes aegypti mosquito, dengue causes flu-like symptoms including high fever, rashes, joint pain and, in the worst cases, haemorrhaging and death.

The World Health Organization estimates that 50 to 100 million infections occur every year, including 500,000 haemorrhagic cases and 22,000 deaths, mostly among children.

Before 1970, only nine countries had experienced severe dengue epidemics, but today about 2.5 billion people, or 40 per cent of the world’s population, live in areas where there is a risk of dengue transmission.

Vietnam is one of at least 100 countries in Asia, the Pacific, the Americas, Africa and the Caribbean, where the disease is now endemic. Since 2000 it has experienced a rise of over 100 per cent in the number of dengue cases due to the failure to control the mosquito population.

In 2017, the country suffered a major outbreak that saw a 51 per cent increase in cases to 184,741, and 32 recorded deaths.

Ms Tsarouchi said that the early warning system would draw from a long history of satellite data covering the whole of Vietnam, forecasts that will predict the weather for the next six months, and from Earth Observation-based information that would include specifics on land mass and population density.

Important variables such as rainfall, temperature, humidity, will also be taken into account.

“The idea is that all of this data is coming together and we develop a digital dengue model based on the historic information to project what will happen in the future,” she said.

The project will be run out of Vietnam’s ministry of health, which already has a dengue operations room and field surveillance capabilities in its department of preventive medicine.

“At the moment the system is very much reactive so if they are able to have some information in advance about a possible outbreak then hopefully they can mobilise their forces and do early campaigns to raise awareness among the local population or do more targeted spraying,” she said.

Public awareness about possible local outbreaks would also encourage people to seek earlier hospital treatment, said Ms Tsarouchi.

Darren Lumbroso, a fellow engineer at HR Wallingford and the dengue project’s leader, added that timely intervention would allow residents in high risk areas to cover water storage areas to avoid mosquito breeding grounds.

He said the project had benefited from health expertise from the London School of Hygiene and Tropical Medicine, the UK Met office, and Oxford Quality Management which carries out monitoring and evaluation.

The consortium received more than £4 million from the UK Space Agency’s international partnership programme.  

The WHO and United Nations Development Fund, as well as local scientific bodies have also jumped on board the unique effort to prevent dengue deaths.

Dao Khanh Tung, a UNDP programme analyst in biodiversity and environmental health, said the UN body would help the authorities to act on the new information and make “more timely decisions for prevention and treatment”.

He added: “Local people need to be aware of dengue outbreaks, of the harm that dengue can cause to their health, and aware of what they can do to prevent dengue fever – cleaning their houses, cleaning up [water] containers and the environment surrounding their households.”

Red Dead Redemption 2 Soundtrack to Be Released

Red Dead Redemption 2 developer and publisher Rockstar Games is planning to release the game’s official soundtrack as a standalone product.

The soundtrack features vocal performances from musicians such as Willie Nelson, Nas, and Josh Homme, as well as a track written and performed by David Ferguson, who previously worked as a sound engineer for Johnny Cash. Vocals in the game were produced by Daniel Lanois, who has produced albums for folk legends like Neil Young and Bob Dylan.

The score is a major collaborative effort. It was primarily composed by Woody Jackson, who worked on the original Red Dead Redemption as well as Grand Theft Auto V and L.A. Noire, with additional production handled by Jeff Silverman of Truth and Soul Records.
